Questions for Patients
- Have you been traveling in the last 14 days?
- Have you recently had a fever or runny nose/coughing or trouble breathing?
- Have you been in contact with anyone with COVID-19 within the last month?
** If you answered "yes" to any or all of these questions, we will not allow you into our office at this time. Please call our front desk within 48 business hours of your appointment to reschedule. If notice is not given in a timely manner, you will be charged for your missed appointment.
Please be patient with us — this is uncharted territory for everyone, and we are implementing specific protocols for the protection of our patients and staff. We need to take extra care to keep our space clean and free of germs and viruses. As many of you know, Dr. Schantz is pregnant, and cannot risk an exposure from a possible known carrier or symptomatic patient. Please reschedule your appointment if you are not feeling well - even if it's just a minor cold - to prevent potentially infecting others. We all need to do our part to keep one another safe and healthy!
